Treatments for Mesothelioma Under Investigation - Law

A double-blind study in Phase II Trial will compare the results of administering the combination of Gemcitabine and Cisplatin with, and without Bevacizumab, a drug that inhibits the growth of blood vessels that can nourish new cancer cells in the hope that the tumor will shrink.

Pemetrexed combined with Cisplatin was compared to Cisplatin alone in a Phase III trial involving patients with metastatic pleural mesothelioma (MPM). The combination of both drugs with the addition of vitamin B12 and folic acid resulted in superior survival time, time to progression, and response rates Adding the vitamins reduced toxicity.

Another study in Phase I involves Pemetrexed and Carboplatin in patients with MPM. During Phase I some of the patients experienced toxicity related to the dosage, however, 70% of the patients experienced improvement in their symptoms after only two courses of treatment. Time to progression was delayed 305 days (about ten months) and median survival time 451 days (about fifteen months.)

Photodynamic therapy is another approach that is currently being studied. Patients are pretreated with light-sensitive molecules that cause the cancer cells to become sensitive to light. During surgery, these cells are exposed to light of a specific wavelength. Though there have been positive results, this treatment also requires more study. In comparison, the patients who qualified for these clinical trials had a life expectancy of six to nine months at the time they entered the trials. While there is still a long way to go, scientists are working to find the key to curing mesothelioma. In the past surgery was done as a palliative treatment.

Fifteen years ago researchers were hopeful that surgery combined with chemotherapy, radiation therapy would provide more significant results than each give separately. Today, therapy combining surgery with chemotherapy, photodynamic therapy and gene therapy are producing encouraging results.

Long Time Mesothelioma Survivors -What Do They Have in Common? - Other

Paul Kraus is one of the long-term survivors of malignant mesothelioma, there are many others and one thing they all have in common to all of them is the fact that they all focused most of their treatment on steps to improve or enhance their immune system. The researchers wanted to know if there were any measurable differences in the immune responses of the mesothelioma patients. Their findings demonstrated a relationship between the immune system and malignant mesothelioma. For example:

* The number of total T (T11+) and T-helper (T4+) cells were normal in asbestos workers with cancer, but were significantly reduced in patients with mesothelioma. T cells orchestrate, regulate and coordinate the overall immune response.

* Most patients with mesothelioma had a profound deficiency in Natural Killer cell (NK) activity which is suggestive of the role the immune system plays in the control of malignant mesothelioma. NK cells are a type of lethal lymphocyte that target tumor cells and protect against a wide variety of infectious microbes.

In the discussion section of the report, the researchers stated:"These findings led us to speculate that biological phenomena generally categorized as chronic immunosuppression associated with the presence of asbestos fibers in the exposed workers may have caused the eventual breakdown of the host's surveillance system and the onset of neoplasm [malignant mesothelioma]."

In other words, the researchers are suggesting that malignant mesothelioma may result from immune suppression. If this is true it would provide the biological basis for the role that the immune system and immune boosting approaches may play in the management of malignant mesothelioma.

Another example of a long time survivor of mesothelioma is James O'Connor.In October 2001, sixty-one year old James Rhio O'Connor ("Rhio") was diagnosed with pleural mesothelioma caused by his exposure to asbestos when he was younger. His was given less than a year to live.

His tumor was not operable because the tumor was too close to his spinal cord chemo was also not an option because at the stage of his tumor chemo would not have been of any major help to him, it could not give any significant elongation to his life span. He was basically told to prepare for imminent death in a polite manner.

To soften this message, the doctor also recommended that Rhio take his wife on a cruise and then start hospice care upon his return. Rhio rejected the idea. He was determined to survive this cancer. Mesothelioma is a rare form of cancer in which cancerous cells are found in the mesothelium, a protective sac that covers most of the body's internal organs. Treatment for Mesothelioma depends upon the location of the malignant cancer, the stage of the illness, and the patient's age and general health. This cancer occurs about four times more frequently in men than in women and all forms of mesothelioma, except for benign mesothelioma, are invariably fatal.

Symptoms of peritoneal mesothelioma may include pain or swelling in the abdomen due to a build-up of fluid, nausea, weight loss, bowel obstruction, anemia or swelling of the feet. Unfortunately, the mesothelioma usually does not result any symptoms or health problems until the disease has greatly progressed.

Sometimes resembling viral pneumonia, pleural mesothelioma patients may present with shortness of breath, chest pain and/or persistent cough; some patients show no symptoms at all. Less common symptoms of pleural mesothelioma include fever, night sweats and weight loss. Typically, symptoms of Mesothelioma do not appear until 20 or more years after exposure to asbestos.

Advanced malignant Mesothelioma disease can again be divided into 3 stages namely stage II, III and IV. The mesothelioma disease usually takes twenty or more years to develop, and people that worked with asbestos in the 1970s may currently start showing signs of disease.
